Project background

Despite a compelling evidence-base housing is an important determinant of health outcomes, implementation is variable. Housing influences admissions to hospital, for example a cold home is a major factor in ‘winter pressures’ from patients with cardio-respiratory illness. A housing adaptation may also help to improve hospital discharges, for example if a patient has some limited mobility. 

The Project

This project therefore attempts to build bridges between housing and health. 

How this will be achieved

This project will deliver three products in the Hywel Dda footprint. Firstly, a front-facing internet page with housing information for the public that we serve. Secondly, the delivery of referral pathways, for example patients accessing our smoking cessation service will be sign-posted to a warm homes project. Thirdly, bespoke performance management tools will also be developed iteratively. 

Anticipated benefits

Whilst recognising the value of a locally sensitive approach to housing, having a good practice template for use across all the Welsh Health Boards would be highly advantageous. At a national footprint, the intention is therefore to liaise with colleagues across Wales to share learning, experience, good practice and challenges. This will also simultaneously raise the profile of housing and health. The desired outcome from this is a national good practice guide for the NHS.
